3. Who is taking patients in Ridge Meadows? Find a Doctor website launch! As part of our A GP for Me Physician recruitment and retention work we are pleased to announce the launch of a new Find a Doctor toollocated on the Division's web site. Working with our member’s respective practices, the Division will maintain a list of family doctors who are accepting new patients publically. Patients looking for a family doctor will be able to find the list at www.findadoctormapleridge.ca or www.findadoctorpittmeadows.ca . We are planning to market the Find a Doctor tool through a broad distribution of media, e news, flyers, business cards and/or postcards throughout the public and the medical community. This is a long awaited tool and a win-win for physicians who are looking to take on new patients as well as for people who are looking for a family doctor. Strategically we are also using this a recruitment tool to attract new GPs. As new GPs plan to arrive in our community to practice, they can start pre building a patient list and arrive ready to begin.

5. Division Project Update I Seniors, Member Education, CYMH, Acute Care , Residential Care and IT Strategy
Acute Care Project As part of the Acute Care project to improve communication between family physicians and hospital-based physicians, the planning committee is facilitating a GP Breakfast learning session: “Demystifying the ER”. The committee will be sending out information soon about this session and a confirmed date which will take place near the end of June. This is an opportunity for family physicians to dialogue with ER physicians about some of the challenges of the ER and how we can work together to serve patients better. You will receive a one hour stipend and breakfast is included. If you are interested in receiving more information about this session please email tmiyashita@divisionsbc.ca
Child and Youth Mental Health The first phase of the CYMH initiative was launched in May with three meetings of the Local Action Team. Participants included local physicians, specialists and highly committed community stakeholders. Two projects were chosen as a result of these meetings:
1. Journey Mapping and Partnership Development
2. Collaborative Care Model Pilot
Upon approval of the projects from Shared Care, Phase 2 will begin in June 2015 and conclude in March 2016. For more information on this project email lgraham@divisionsbc.ca
Seniors Project The Division applied for Shared Care funding to begin a seniors initiative and was successfully awarded funding. The project’s main goal is to develop a collaborative care model that involves family physicians, specialists, home/community support and patients. This initiative kicked off in February with a stakeholder meeting to identify key challenges facing seniors care in our community. It is evident we have committed stakeholders and seniors that want to work with physicians to improve care and make a difference for our senior population. We look forward to updating you further as this project unfolds and launches in June.
Residential Care Initiative The Residential Care Initiative is a provincial strategy to improve care of patients living in residential care by providing financial benefit and resources for physicians. Physicians must be willing to meet expectations such as being part of a rotating on-call group that will serve the residential care facilities, respond proactively to nursing issues, conduct meaningful medication reviews, be involved in case conferences, and complete documentation. If you are interested in joining this planning committee which will be approx. 3 meetings over the next four months, please click here or contact Dr Martin Milewski.
IT Strategy The Division will soon be launching a clinic site visit program. If you have previously been a peer mentor through the former PITO program delivered by Doctors of BC and are interested in continuing to be involved in delivering EMR support to your peers, please click here. This Division will be facilitating an orientation meeting for all peer mentors to kick off this new program model. Details will be coming in the June digest for clinics wishing to book a support site visit.
